Faith Community Nurse

The Faith Community Nurse program is provided by Penrose-St. Francis Mission Outreach and sponsored by Tri-Lakes HAP.

The role of the Faith Community Nurse is to provide health education, acting as a personal health care counselor and advocate.

The Faith Community Nurse provides a variety of services, including: blood pressure screening, macular degeneration / low vision support group community resources for health referrals and pharmaceutical assistance.
