Pediatric Immunization Clinic

The Ronald McDonald Care Mobile rolls into the Tri-Lakes area to provide no cost immunizations for those who have no insurance or are on Medicaid or CHP+, or who have a private insurance plan that is accepted by Peak Vista.

Clinic Dates and Times:
Immunization Clinic is at the Tri-Lakes Cares building, 235 Jefferson, in Monument, and runs from 1–5 PM .

Bring the child's immunization record when attending the Immunization Clinic.

During the Immunization Clinic volunteers will be available to help people apply for Medicaid, CHP+. If interested in enrolling in any of these programs please bring your child's birth certificate, the child's school ID card with photo or driver's license or passport, the child's social security card, and at least one paycheck stub from the current or previous month.
